DESCRIPTION:A puzzling part of anorexia nervosa (AN) is the maladaptive re
 sponse to negative energy balance\, i.e.\, maintained starvation and low b
 ody weight. Large-scale genetic studies suggest AN is correlated with seve
 ral metabolic and anthropometric traits\, and genetic variants being enric
 hed in microglia. Hypothalamic circuits are fundamental regulators of ener
 gy balance\, and emerging evidence indicates that interactions between hyp
 othalamic neurons and microglia are central to this energy homeostasis. We
  hypothesize that a genetic vulnerability in AN results in dysfunction of 
 hypothalamic microglia.\n\nWe generated fibroblast-derived induced pluripo
 tent stem cells from AN patients and healthy controls (HC)\, and different
 iated them into microglia\, hypothalamic and cortical neurons. Synaptosome
 s isolated from the neurons were fed to microglia\, assessing their phagoc
 ytic capacity. Additionally\, we performed spatial transcriptomic profilin
 g of hypothalamic microglia in anx/anx and wild-type mice.\n\nAN-derived m
 icroglia exhibited significantly reduced phagocytosis of hypothalamic syna
 ptosomes compared to HC\, while no difference in phagocytic capacity was o
 bserved with cortical synaptosomes\, indicating selective aberrant synapti
 c engulfment of hypothalamic circuits in AN patients. Spatial transcriptom
 ics of hypothalamic microglia revealed differentially expressed genes cent
 ered around synaptic pruning in the anorectic anx/anx mouse.\n\nTaken toge
 ther\, our findings suggest that aberrant microglia-driven synaptic prunin
 g of hypothalamic neurons contributes to the maintained negative energy ba
